Berkisah tentang seorang gadis bernama Madoka Kaname, usia 14 tahun, mengalami sebuah mimpi buruk dan ditawari menjadi Magical Girl (Mahou Shojo) bersama dengan temannya Sayaka Miki oleh Kyuubey. Teman dari Kyuubey, seorang magical girl bernama Mami Tomoe pun mengharapkan mereka juga menjadi seperti dirinya. Tapi, dengan alasan tersembunyi, seorang magical girl lain bernama Homura Akemi bertekad untuk melarang Madoka menjadi magical girl. Apalagi ada sesuatu yang misterius di balik kontrak tersebut.....
